H2U Wellness Centers’ Physician, Nurse Practitioner and other medical staff will take the time to provide you with information concerning your medical care, and encourage your participation in healthcare decisions. Therefore, your personal health information will be treated confidentially and will not be released to anyone without your prior written consent.
