Skyward Student Online Forms
Please follow the instructions below to complete this survey in Skyward.
1) Login to Skyward Family Dashboard 2) Click on the Education Benefits Survey 3) Complete these steps for EACH student
The Texas Education Agency requires school districts to annually collect student information, health information, and emergency contact information. Parents/guardians of all new and returning students are required to provide this information by completing online registration using Skyward Family Access.
ALL ONLINE FORM NEED TO BE COMPLETED BEFORE SCHOOL BEGINS ON
AUGUST 14, 2025.
Parents of all new and returning students are required to complete all online forms starting July 24, 2025.
Student-specific online registration documents, which includes the name, username, and email address of the parents/guardians recorded in Skyward under Family 1, will be sent home the first day of school, but you can complete the annual verification NOW!
Steps to completing the online forms for all students for 2025-2026.
- Log into Skyward Family Access
- Click on each of the online forms to complete.
IMPORTANT THINGS TO KNOW:
- Items requiring an answer are marked with a red asterisk. (*)
- You may stop the process and finish it at a later time.
- You must click “submit” at the end of the process for your responses to be recorded and sent to the campus for approval.
- Once the campus approves your documents, your responses will be reviewed to determine if your child qualifies for any additional programs and the emergency contacts you provided will be updated in Skyward.
- IMPORTANT: Once the online form is submitted, do not re-open the form as it will undo the submission.
