The original affidavit must be on file in the student's health file. Copies of the affidavit are not acceptable.

Each individual vaccine exemption affidavit is good for two years from the date notarized.

Obtaining a vaccine exemption form for reasons of conscience may be obtained by parents or guardians. The written request must be submitted through the U.S. Postal Service, commercial carrier, fax or via a secure online request form.
