Immunization Requirements

A student must be fully immunized against certain diseases or must present a certificate or statement that for medical reasons or reasons of conscience, including a religious belief, the student will not be immunized.

  • For a medical expemption to state-required immunizations, you will need to submit a statement signed by the treating physician licensed in the state of Texas noting which immunizations the student shall be exempt from. Unless otherwise stated in the physician’s statement, medical exemptions must be renewed annually.
  • For exemptions based on reasons of conscience, only official forms issued by the Texas Department of State Health Services (DSHS) Immunization Branch will be honored by the district.

The original copy of the notarized exemption affidavit must be submitted to the school prior to enrollment. If the parent is seeking an exemption for more than one student in the family, a separate form must be provided for each student. The affidavit will expire after 2 years from date of notory signature/date.
